Description (Service) Attendant care for adults living with a physical disability, in their own apartment, within a supportive housing complex * customized attendant care service hours to meet clients' individual needs
Hours Mon-Sun 24 hours
Areas Served Lanark County ; Leeds and Grenville United Counties
Eligibility Ages: 16 year(s) and up 
 
Individuals 16 years and older with a permanent physical disability, the ability to direct their own care, a valid Ontario health card and residence in Ontario * not in receipt, or not expecting to be in receipt of, insurance payments or benefits for attendant services
