Description (Service) Delivers one-on-one care to people in their own homes, schools or workplaces * helps with daily activities such as taking medication, personal grooming and hygiene, rising and retiring, dressing and undressing, bathing and washing, transferring, toileting, bowel/bladder services, range of motion exercises and essential communication tasks
Hours Administration Mon-Fri 8:30 am-4:30 pm
Areas Served Lanark County ; Leeds and Grenville United Counties ; Renfrew County
Eligibility Ages: 16 year(s) and up 
Individuals who: 
* are 16 years and older 
* are able to direct their own care 
* own a valid health card 
* live in Ontario 
* have a permanent physical disability and are not in receipt of insurance funding designated for attendant care services
