Description (Service) Health centre focused on family and community health * clinics located in Lanark, Carleton Place and Smiths Falls 
 
Services: 
* Prenatal, well baby, child and adult care, including immunizations 
* Nutrition counselling 
* Individual, family and couple counselling 
* Speech language services children 0-5 years or age 
* Community presentations and workshops 
 
Staff includes: Nurse Practitioner (Primary Health Care) * Registered Dietitian * Social Work Counselors * Speech-Language Pathologist
Sup. Description Weekly clinics at the North Lanark County Community Health Centre 207 Robertson Dr in Lanark Village * Weekly clinics in Carleton Place at 30 Bennett St * Weekly clinics at Smiths Falls Community Health Center (Nurse practitioner only) * Workshops, groups and presentations throughout Lanark County
Hours Mon-Fri 8:30 am-4:30 pm, evening hours available * some programs and services offered weekday evenings
