Description (Service) Program can help to buy, repair and maintain a wide variety of mobility or assistive equipment
Hours Mon-Fri 9 am-4:30 pm
Areas Served Ontario
Eligibility Ages: 19 year(s) and up 
Permanent Ontario residents 19 years and older with physical disabilities who are in financial need to purchase assistive devices to increase their mobility and functional independence.  
The after-tax income of both the applicant and his/her spouse/life partner are considered. Maximum allowable income may change based on family size (calculated using after-tax household income at or below the Statistics Canada Low-Income Cut-off + 15%) 
Family size maximum income allowed: 
* one person: $22,920 
* two people: $32,413 
* three people: $39,698 
* four people: $45,839
