Description (Service) Rehabilitation and support services for adults with moderate to severe acquired brain injuries. Services include individual and / or group activities in clients home or community.

Eligibility Ages: 16 year(s) and up 
Individuals 16 and older with a confirmed diagnosis of acquired brain injuries
