Description (Service) Compassionate and comprehensive care of the terminally ill and their families at a time when treatment aimed at cure and prolongation of life is no longer appropriate * the End-of-Life Care unit consists of ten private rooms and two fully-furnished family rooms * needs assessment to provide a holistic program of care * bereavement support and assistance to family and friends

Areas Served Frontenac County ; Kingston ; Lennox and Addington County
Eligibility Ages: 16 year(s) and up 
Individuals 16 years and older with a prognosis of less than four months of life and who require symptom management
Application A referral from the family physician or a health care agency is required * the patient is assessed prior to admission by the physician in charge of the unit and by a nurse * forms available on the website for short-term palliative care and long-term palliative care
