Description (Service) Home visiting program for pregnant women and families with children from birth to transition to school * our home visiting team consists of registered nurses and family home visitors (parenting peer support) * all parents and families referred will be screened initially * screening provides opportunities for early identification and support options for families through home visiting, service coordination, and referrals to services to support healthy child development
Hours Mon-Fri 8:30 am-4:30 pm * closed satutory holidays
Areas Served Frontenac County ; Kingston ; Lennox and Addington County
Eligibility Pregnant women and families with children from birth to their transition to school
Application Individuals may self-refer, or referrals may be made by community organisations with client consent * visit the website for referral forms * call the Child and Baby Talk phone line
Languages English - Interpretive or translation service will be used for other languages.
