| Description (Service) | Association of family members, people with disabilities and interested community members, supporting people in Leeds and Grenville Counties with an intellectual disability * affiliated with Community Living Ontario and the Canadian Association for Community Living Family Support Coordination Program: individualized supports including information and referral, in-home support, family relief/respite, assistance with participation in valued roles in community life (leisure, friendship, employment, education, etc.), assistance with applications to government for funds or special assistance, coordination of services, planning for future Host Family Program: matches people with disabilities with families in the community * provides respite and gives the person with a disability a chance to get to know other people and try different experiences |
