Description (Service) Diabetes management clinic * education and support for people living with diabetes, other chronic illnesses and their families
Hours Mon, Wed, Fri 8 am-5 pm * Tue, Thu 8 am-7 pm * regular diabetes clinics available once per week in three locations: Portland, Westport, and Elgin
Areas Served Elizabethtown-Kitley - parts ; Leeds and the Thousand Islands - rear of Leeds and Landsdowne ward only ; Rideau Lakes ; South Frontenac - parts ; Westport
Eligibility Ages: 18 year(s) and up 
Individuals 18 years and over with diabetes * clinic is for clients of Country Roads Community Health Centre only * Group Cooking and Self Management Group are open to full community
