Description (Service) A program to assist seniors, who are at risk of loss of independence, to remain in their homes * provide access to services including housekeeping, laundry, meals, respite, seasonal outdoor chores, shopping and transportation * improves seniors quality of life

Eligibility Ages: 75 year(s) and up 
To be eligible, the senior must meet the following criteria:  
- Be at risk of admission to hospital or a long term care home. 
- Require assistance with multiple household tasks and activities due to either a functional or cognitive impairment. 
- Be over 75 years of age or living with a disease of aging. 
- Live alone or with a caregiver unable to assist.
