Description (Service) Specialized clinic completing comprehensive assessments, interventions, and health teaching for patients at risk of and following a stroke and/or transient ischemic attack.
Hours Mon, Wed, Fri 8 am-4 pm * may vary with statutory holidays
Areas Served Lanark County ; Leeds and Grenville United Counties
Eligibility Patients at risk of a stroke and/or following a stroke or transient ischemic attack (TIA)
Application Physician referral required * bring a valid health card to every visit * bring complete list of medications and health history to each appointment
