Description (Service) Provides primary health care services, education and health promotion including chronic disease prevention and management programs * teen health clinic * support groups: depression, diabetes * interdisciplinary team * volunteers

* Early Years (including breastfeeding support) 
* Good Food Box 
* Community Kitchen 
* Walking and fitness programs  
* Youth (after-school programs, cooking sessions, childcare training and summer program)

Meetings Teen health clinic: twice a week, Rideau District High School
