Description (Service) Home support program to help seniors and physically disabled adults to remain in their homes and community * Serves Perth, Smiths Falls, Merrickville, Maberly, Lanark,and Carleton Place. 
Programs include: diners club * transportation * Meals on Wheels * foot care clinics * Parkinson's support group * home help *  
hospice/palliative care, respite service
Hours Mon-Thu 8:30 am-4 pm, Fri 8:30 am-3:30 pm
