Areas Served Lanark County ; Ottawa (City of) - Ottawa Valley
Eligibility Ages: 3 year(s) and up 
Residents of the Ottawa Valley 3 years and older, who are physically and/or developmentally handicapped, or at risk for being developmentally delayed
Application Riders must fill out a referral form and make arrangements for a home visit where the rider is assessed, release forms are filled out and the program is explained. * Medical Doctor signature required on permission forms at the onset and each three years thereafter.
