Description & Services
Description (Service) Provides community-focused mental health services to older people in Leeds and Grenville counties who are suffering from a psychiatric disorder such as depression or dementia, with resulting behaviour problems * team members provide diagnostic assessment and consultative services in private homes and health care facilities; monitor psychotropic medication; find and arrange resources; provide support and health teaching to clients, family and caregivers; liaise with community services providers; provide consultation, training and education to community agencies serving the elderly
