Description & Services
Description (Service) Helps adults who have a developmental disability and who live on their own in the community. Case managers help individuals find the right supports to be able to have more independence and choice. Can help plan for the future and link to supports and services.
Hours Mon-Fri 8:30 am-4 pm, by appointment
