Description (Service) Early identification and prevention program * pregnant women and families with young children who are identified as having stresses are offered home visits by a public health nurse and family resource worker. 
 
Children age six weeks to six years can be screened to determine if they are reaching their developmental milestones. * Information on children's services and referrals to community agencies are available. * Some topics discussed during home visits include: healthy lifestyles, child development, breastfeeding, and parenting
