Champlain Going Home Program, Ottawa site: referring patients at time of discharge after hospital stay or release from the Emergency Room * assistance for up to 10 days after referral for:
* Meals on Wheels
* transportation home
* follow up appointments
* homemaking
* information on community support services in local community for seniors
