To be eligible for the voucher services, a client must partner with a social worker, a hospital, a church, a shelter, and other similar agencies and ask them to write a referral letter. The referral letter should mention that the patient is in financial and visual need of eye glasses. The client must then bring their prescription for eyewear, their referral letter, and the referring agency's Charitable Registration Number to a local participating LensCrafters, Pearle Vision, or Sears Optical location.
