Description & Services
Description (Service) Team of mental health professionals who provide specialized services to persons with severe and persistent mental disorders * accompanying services as well as support during the person’s daily life 
Services: symptom evaluation and control * pharmacotherapy * individual therapy * integrated mental health and drug abuse treatment * home support * assistance in organizing daily life * educational support * employment support * socialization and leisure activities * ability development * crisis intervention
Hours Mon-Fri 8 am-4 pm
Areas Served Ottawa (City of)
Eligibility Ages: 18 year(s) and up 
Persons 18 years and older with chronic and/or persistent mental illness * also, persons who have frequent or prolonged admissions to hospitals for mental health reasons 
Persons who have a significant and apparent functional disorder that is defined as: the inability to keep up with daily activities, to ensure personal safety and to participate in existing services
Application By referral through the Central Intake Officer at 613-722-6521 ext 7325.
