Description & Services
Description (Service) Provides specialized social work services and interventions as an integral part of the interdisciplinary family-centered patient care approach: 
* emergency department 
* outpatient clinics 
* inpatient services 
* psychosocial assessment and counselling intervention to help patients and families effectively cope and adapt to the impact of trauma, illness and hospitalization 
* advocacy on behalf of patients and families for necessary community resources
Hours Mon-Fri 8 am-4 pm * after hours services available for urgent situations, during evenings, weekends and statutory holidays
Areas Served Frontenac County ; Hastings County ; Lanark County ; Leeds and Grenville United Counties ; Lennox and Addington County ; Ottawa (City of) ; Prescott and Russell United Counties ; Prince Edward County ; Qu├ębec - Western ; Renfrew County ; Stormont, Dundas and Glengarry United Counties
Eligibility Ages: 18 year(s) and under 
Patients 18 years and under and their families
Application Referrals for individual, group and family counselling are received from members of the health care team or from families through a referral by a physician
