Description (Service) Provides intensive services to the severely mentally ill people * comprehensive assessment (psychosocial, psychiatric) and treatment planning * psychiatric consultation * individual and group therapy * multidisciplinary staffing * case management chronic clinics (outreach services and medication monitoring clinics) for the hard-to-serve psychiatric population
Hours Mon-Fri 8 am-4 pm
Areas Served Akwesasne ; Cornwall ; Stormont, Dundas and Glengarry United Counties
Eligibility Ages: 16 year(s) - 65 year(s) 
 
Adults and youth 16 years and older with a serious mental illness * older adults 65 years and older will be forward to a psychogeriatrics team
