Description (Service) Access point for prediabetes and type 2 diabetes community education * intake and referral * individual and group sessions offered by registered dietitians and registered nurses * services include lifestyle management, insulin initiation and medication/insulin adjustments * education sessions are suitable for both newly-diagnosed adults and refresher courses for adults living with diabetes for years; family members are welcome
