Description (Service) Integrated community health and social services * confidential and personalized addiction and concurrent disorders services for individuals concerned about their use of alcohol, drugs or gambling and co-occurring mental health concerns*  
 
Assessment * treatment planning * referral * support * follow-up appointments * out-patient counselling for clients experiencing barriers to access community services
