Public Comments Fraudulent Automated Calls to Patients: some home care patients have received fraudulent automated telephone calls informing them they would be discharged from home and community care services, but giving them the choice to purchase additional services. Description (Service) Coordinates all admissions into long-term care homes (including short-stay and respite care beds) and into complex continuing care (chronic care) facilities across Champlain * refers placement applications and assists in the coordination of admissions of long-term care facilities in other regions of Ontario

Hours Mon-Sun 8 am-8 pm
Areas Served Lanark County ; Leeds and Grenville United Counties ; Ottawa (City of) ; Prescott and Russell United Counties ; Renfrew County ; Stormont, Dundas and Glengarry United Counties
Boundaries The boundaries for the Champlain CCAC are the same as those of the Champlain LHIN which include Renfrew County * the City of Ottawa * Prescott and Russell * Stormont, Dundas and Glengarry * North Grenville * four parts of North Lanark (Beckwith, Carleton Place, Mississippi Mills, and Lanark Highlands).
Eligibility Referrals for placement services are accepted from institutions, agencies, physicians, hospitals, individuals in the community * Professional staff visit, counsel and assess referred persons and determine if they meet the eligibility criteria for placement/admission to a long-term care facility
Application Individuals can phone directly or they can be referred by family, friends or another professional
Languages English ; French
Fees Service ; Placement is free. The basic accommodations rates are set by the Ministry of Health and Long-Term Care and some subsidies are available. There is a cost for long-term or respite care in a long-term care facility bed and a cost for a complex continuing care hospital bed as well.
