Description (Service) Supports for people living with pre-diabetes or Type 2 diabetes 
 
* team includes a nurse and a dietitian 
* group education sessions 
* individual follow-up 
* insulin initiation 
* information on nutrition 
* exercise 
* glucose monitoring 
* stress and emotions 
* preventing complications 
* foot care and other self-care practices
