Description (Service) Registered Nurses (RN) with extensive knowledge and experience assist patients and families as their Care Manager for both acute situations and chronic conditions 
 
Holistic approach may include: 
* accompaniment to medical appointments 
* oversight of treatments and procedures  
* communication with physicians and other health care providers 
* assistance with access to community resources 
* research diagnoses and treatment options 
* investigation of alternatives based on patient's needs
