Description & Services
Description (Service) Helps maintain seniors of the Ottawa community in their home by managing challenging behaviours, and implementing treatment strategies to avoid unnecessary hospitalization and placement. * Staff work in collaboration with the family physician. 
Services: assessment * treatment * counselling * education * support in cases where the suspected abuse is related to clients 65 years of age and over who have mental health problems and/or dementia 
Interdisciplinary team includes: geriatric psychiatrists * registered nurses * occupational therapists * social workers * capacity assessors * psychogeriatric resource consultant 
Core functions: intake and screening process * specialized client centered assessment * case management * diagnosis * care planning and treatment * counselling * family and caregiver education and support * behavior management * client advocacy 
Geriatric psychiatrists provide: diagnosis * consultation * treatment * support to professionals in the community
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Ottawa (City of)
Eligibility Ages: 65 year(s) and up 
Persons 65 years and older with mental health problems or suffering from dementia, and persons less than 65 with Alzheimer's dementia or Frontotemporal Dementia complicated by behavioural or psychological symptoms. 
Clients living in long-term care homes are excluded from services.
Application Signed referrals are received from physicians and health care professionals.
