Description (Service) Intensive treatment services in mental health in clients' home 
 
* assistance in finding suitable housing 
* medication management 
* help to reduce harm caused by addictions 
* promotion of community integration 
* help to find other health care services such as a physician or therapist 
* peer support 
* development of personal interests and leisure activities
Hours Mon-Fri 8 am-4 pm
Areas Served Prescott and Russell United Counties
Eligibility Ages: 16 year(s) and up 
 
Individuals 16 years and older with schizophrenia, bipolar disorder, or other severe, persistent, and/or recurrent mental health problems
Application Self-referrals accepted * physicians, health care professionals, and community organizations that dispense health care services may also refer clients * for all new referrals, call central intake, toll free 1-844-304-1414
