Description (Service) Medical Assessments & Therapeutic Treatments provided in private home settings, public and private residential and nursing homes. Treatments are provided by Occupational Therapists, Kinesiologists, Registered Dietician, Psychotherapists, Clinical Psychologists, Registered Nurses * works with professionals in Physical Therapy, Speech Therapy, Medical Care, Family Physicians, Social Workers, and psychological counselling 
* occupational therapy in home assessments 
* home-based safety assessment 
* chronic pain assessment 
* functional capacity evaluation 
* future care cost assessments 
* ergonomic workspace assessment 
* standard musculoskeletal assessment 
* Assistive Devices Program (ADP) assessment for manual/power wheelchairs, walkers, scooters and communication aids 
* attendant care needs assessments 
* housekeeping needs assessments 
* cognitive and physical demand evaluation 
* group therapy sessions
Hours Mon-Fri 9 am-7 pm
Areas Served Hawkesbury ; Ottawa (City of) - and surrounding communities
Eligibility Individuals suffering from injuries, illnesses and long-term disabilities, or who have had a motor vehicle accident
Application Call or walk in * referral required for clinical services covered under private insurance or funded provincial programs where applicable
Languages English ; French ; Arabic ; Farsi ; Russian ; Spanish ; All languages can be accommodated via interpertation services
Fees Service ; Medical fees covered by direct billing to insurance companies * direct billing also available for members of Veterans Affairs Canada (VAC) , Canadian Arm Forces (CAF) and members of the Royal Canadian Mounted Police (RCMP) through health care benefits provided through Medavie Blue Cross.
