Description (Service) Program dedicated to people having a terminal illness and their family * managed by a registered health care professional assisted by a team of trained volunteers 
Day program: support group for emotional and social support * hot lunch and snacks * activities: discussions about concerns and issues related to illness, treatment, and other topics, music, storytelling, games and puzzles, guest speakers * services: social worker, reflexology, reiki, massage, zootherapy * quiet room 
Home visits: helps clients remain at home or at long-term care home * supportive care * respite for family * bereavement follow-up * outings * volunteer opportunities
Hours Day Program: Wed 9:30 am-2:30 pm 
Home visits: Irregular, call for details
Areas Served Prescott and Russell United Counties
Eligibility Ages: 18 year(s) and up 
Individuals 18 years and older with a terminal illness or life limiting disease * definitive medical prognostic not required
Application Clients: registration required * call to book appointment * coordinator will do evaluation at home or long-term care center * referral not required 
Volunteers: call the Coordinator * police check required * training offered * minimum commitment: 4 hours weekly * currently looking for volunteers in these fields: music, storytelling, massage, reflexology and zootherapy
