Description & Services
Description (Service) Support for youth struggling with mental health and/or addiction issues * connects youth to needed community supports and services such as: addictions, mental health, housing, education, employment, finances, health care, food, life skills and social/recreational * helps youth to transition from one mental health service to another (e.g. from Open Doors for Lanark Children and Youth to Lanark County Mental Health or Lanark Leeds and Grenville and Addictions and Mental Health).

Hours Mon-Fri 8 am-12 noon, 1 pm-4 pm (Smiths Falls) * Perth and Carleton Place: by appointment only
