Description (Service) Note (25 Mar 2020): Due to COVID19, PQCHC will be closed with the exception of its medical clinic. This program is still available in a limited capacity and by phone only. 
Support services to families who have been recently made homeless, or face ongoing challenges in maintaining their current housing * in-home support * families identify challenges and priorities with a worker and are connected with existing resources to help them maintain housing * support is offered over nine months 
Partnership with the City of Ottawa * caseworkers operate out of the following community centres: 
* Nepean, Rideau and Osgoode Community Resource Centre 
* South-East Ottawa Community Health Centre 
* Vanier Community Service Centre 
* Somerset West Community Health Centre 
* Pinecrest-Queensway Community Health Centre

Eligibility Individuals who are homeless or vulnerably housed
Application Referral needed * families who are currently homeless and accessing the City of Ottawa’s Family Shelter Services through the Carling Family Shelter and the Forward Family Shelter may be referred by shelter staff * families living in the community who are vulnerably housed and require support may also be referred through Housing Help and Action Housing
