Description (Service) Diabetes prevention and education centre * group classes for type 2 diabetes and pre-diabetes * individual assessment and follow-up appointments with Certified Diabetes Educators to review personalized eating and exercise plans, suggestions for diabetes medication, and information on insulin * weekly walking group.
