Description (Service) Health centre focused on family and community health * teaching clinic of the University of Ottawa's Family Medicine Program * partnership with Bruyère Continuing Care 
 
Specialized clinics and services include: 
* Mental Health (access to a psychiatric team for short term interventions and prevention) 
* Chronic Disease Management (systematic management of chronic disease to ensure best practices, education in self care, comprehensive care and prevention) 
* Procedures clinic 
* Smoking cessation 
* Diabetes education 
* Diagnostic Imaging (x-ray)
Hours Mon-Fri 8:30 am-5 pm * 24 hour urgent care available for clinic patients through main phone line
