Description (Service) Providing in-home assessment * recovery * treatment planning * support counselling * life skills * teaching * advocacy and liaison with other services

Eligibility Ages: 16 year(s) and up 
 
Case management services are available to people 16 years and over who have been diagnosed with a mental illness including  
* schizophrenia  
* manic depression  
* chronic depression  
* severe anxiety disorder  
* OCD  
 
Client must also have difficulty managing daily life and have experienced long term difficulties with the illness
