Description (Service) Access to health care support at home or in the community in a culturally sensitive manner 
* intake & elgibility 
* client assessment 
* case management 
* referrals to Algonquins of Pikwakanagan Home Care 
* referrals toChamplain Community Care Access Centre (CCAC) for  
* home nursing, * occupational therapy, * physiotherapy 
* provides patient advovcay as requested by clients for appointment, etc 
* maintains an equipment lending system for medical or equipment supplies on a short term basis for home and community care and home care clients
Hours Mon-Fri 8:30 am-12 noon, 1 pm-4:30 pm
