Description & Services
Description (Service) Supports for people living with prediabetes or diabetes 
 
Education Clinics: certified diabetes educators, including registered dieticians and nurses * work in cooperation with physicians, pharmacists and family members * individual and group appointments * referrals and telehealth sessions with specialists for advanced care 
 
Education on: 
* diabetes control 
* healthy living and eating 
* hyperglycemia (high blood sugar) 
* hypoglycemia (low blood sugar) 
* common emotions 
* physical activity 
* medications 
 
Outreach Sites: screening assessments to determine risk of developing diabetes * individual and group education and counselling 
 
Renfrew County Pediatric Diabetes Education Program: at Pembroke Regional Hospital or through Telehealth * in partnership with the Children’s Hospital of Eastern Ontario (CHEO)
Hours Mon-Fri 8 am-4 pm, evening scheduling is possible
Areas Served Renfrew County - and surrounding area including Western Quebec: Chalpeau, Fort Coulonge, Shawville
Eligibility 19 and over * 18 and under for pediatrics
Application Self referral or physician/healthcare provider * call and leave a message to register, response within two business days
