Description (Service) Case management services are available to people 16 years and over who have been diagnosed with a mental illness i.e.  
* schizophrenia 
* manic depression 
* chronic depression 
* severe anxiety disorder 
* obsessive–compulsive disorder (OCD)
Hours Mon-Fri 8:30 am-4:30 pm

Eligibility Ages: 16 year(s) and up 
16 and older diagnosed with mental illness experiencing problems managing daily life and have experienced long term difficulties with the illness
