Description (Service) Supports for people living with prediabetes or diabetes 
Education programs are offered to improve diabetes control and help with health living * group classes are held on three occasions monthly * individual appointments are offered on remaining week days * monthly telehealth clinics with an Endocrinologist

Description Program is delivered across 5 hospital sites and 15 outreach sites in Renfrew County including * Pembroke Regional Hospital * Deep River and District Hospital * St. Francis Memorial Hospital * Renfrew Victoria Hospital * Arnprior and District Memorial Hospital * Arnprior Villa in Arnprior *Quail Creek in Renfrew * Health Centre in Calabogie * Caressant Care in Cobden *Rainbow Valley Community Health Centre in Killaloe * Whitewater Bromley Community Health Centre in Beachburg * Algonquins of Pikwakanagan Family Health Team in Golden Lake * Petawawa Centennial Family Health Team * Petawawa Seniors' Apartments * Fairfield’s Eganville & Area Residence in Eganville * Supple's Landing,Heritage Manor * Riverview Heights retirement residences in Pembroke * Miramichi Lodge and Marianhill long term care homes in Pembroke * total catchment area includes approximately 100,000 residents in Renfrew County
