Description (Service) | Connects individuals to community health care services * information and referral for health services at www.southeasthealthline.ca or by telephone * coordinates professional home health services and personal care support * when needed, will connect individuals with organizations to better meet their needs * if unable to live safely at home with any combination of community services helps investigate living alternatives such as supportive residential options, retirement homes, long term care homes, convalescent and respite care * access point to Long-term Care Home admission * Care Connectors help people without family physicians obtain a family health care provider |

Eligibility | Information and Referral - No restrictions In-home or in-clinic - a valid Ontario Health Card and a medical condition that can be adequately treated in home or clinic as assessed by a Care Coordinator * There are some restrictions as outlined in the Long Term Care Act and provincial guidelines. Care Connect - if you do not have a family Doctor Long Term Care Home admission - assessed by a Care Coordinator |
Application | By telephone (ask for the Access Team), drop-in or written request * LHIN staff are also available in hospitals across Ontario to help plan for patients' return home |
